In any process of change, long-held biases among colleagues can slow down acceptance and adoption. Science knows why: When someone has a long-held belief, they are unlikely to accept information that contradicts that belief. We're all vulnerable to this.

Medication errors are almost always preventable, yet too many still result in adverse drug events (ADEs), estimated to account for nearly 700,000 emergency department visits and 100,000 hospitalizations each year. According to the U.S. Department of Health and Human Safety, ADEs impact nearly five percent of hospitalized patients, making them one of the most common types of inpatient errors.
